Responsibilities

• Guide patients through onboarding, treatment progression, and ongoing care.
• Educate patients on treatment options, timelines, and expectations.
• Ensure patients remain engaged and consistent with their care plans.
• Celebrate patient progress and reinforce long-term success.
• Build strong, trust-based relationships with patients.
• Address patient questions, concerns, and barriers to adherence.
• Provide clear, simple, and proactive communication at every stage.
• Ensure patients feel supported, informed, and confident in their care.
• Manage patient accounts, subscriptions, and documentation.
• Coordinate with clinical and administrative teams to ensure seamless care.
• Support medication ordering, tracking, and follow-up.
• Monitor patient activity and re-engage as needed.
• Partner with PRCs and clinical staff to ensure a consistent patient experience.
• Support clinic flow when needed to maintain efficiency and patient satisfaction.
• Communicate effectively across teams to ensure alignment and continuity of care.
• Take ownership of the patient journey and ensure no gaps in communication or care.
• Maintain professionalism, positivity, and confidence in every interaction.
• Communicate clearly, set expectations, and follow through consistently.
• Collaborate with team members with a shared focus on patient success.
• Contribute to clinic performance through engagement, accountability, and teamwork.
