K Health in New York, NY seeks a Care Concierge Agent to provide white-glove patient support within the Clinic Team. You will coordinate appointments, guide treatment options, and manage provider schedules to ensure smooth, high-quality care.

Must be flexible including nights, weekends and some holidays. Responsibilities include onboarding patients, maintaining accurate records, multi-channel communication, and HIPAA-compliant handling of sensitive information.

Qualifications

• 1+ years in Healthcare/Healthcare startup experience
• Empathetic, trustworthy, and passionate about patient care
• Organized and flexible: handle changing needs with ease
• Experience with Epic, G-Suite, and Slack
• High school diploma; associate's/bachelor's in healthcare preferred

Responsabilités

• Develops close relationships with and serves as primary point of contact for patients
• Onboards patients and validate insurance and IDs within department SLA requirements
• Schedule appointments, follow-up visits and support the patient in navigating care
• Maintain detailed records of patient interactions and progress across systems
• Communicate effectively with patients, providers and co-workers to facilitate care
• Handle multi-channel patient communication (phone, chat, SMS, email)
• Initiate outreach and missed appointment procedures
• Maintain confidentiality per HIPAA
• Act as an expert on Clinic App and related systems
• Monitor provider schedule adherence within SLAs
