In this role, you'll enhance patient well-being by providing personalized support and ensuring seamless care coordination.

To be successful, you should have:

  • A high school diploma or GED is required; one must type at least 45 WPM and possess basic computer skills.

  • Solid verbal and written communication skills; must be able to read and interpret professional documents and draft reports.

Your responsibilities as the Patient Coordinator will include:

  • Respond to referrals, verify patient demographics and insurance eligibility, and assess the organization’s ability to meet patient needs.
  • Obtain and track required physician documentation and secure insurance authorizations per policy.
  • Engage with patients/caregivers to determine supply needs; communicate with referral sources regarding case status and alternative resources.
  • Document all communications, process updates in the system, and ensure timely delivery setup.
  • Work with internal teams and external partners to resolve issues, support quality standards, and follow company policies and procedures.
