Job summary

Summit Orthopedics is seeking a Patient Coordinator to manage clinical support coordination, focusing on scheduling follow-ups, surgery and clinic visits, and cross-team communication. This full-time float role operates primarily in Minnesota with potential coverage at outreach sites.

Under supervision of the Practice Development Manager, you will support patient care, enter orders into the EMR, and handle non-clinical tasks as assigned.

Qualifications

  • High School diploma required.
  • Associates of Medical Assistant preferred.
  • 2 years of related experience or an equivalent combination of coursework and experience.

Responsibilities

  • Answer and respond to patient phone calls, manage team voicemail and email, and triage patient calls daily.
  • Schedule and manage physicians’ daily schedules, clinic and surgery schedules, including urgent needs.
  • Coordinate patient care between team members and service lines across Summit.
  • Communicate information from physicians to patients and staff per protocols.
  • Enter physicians’ orders into electronic medical records accurately.
  • Complete standard patient forms such as FMLA, disability and workers’ comp as directed.
  • Act as back-up to other teams and assist with voicemail, email and tasks as needed.
  • May room patients and initiate visits as directed by providers.

Under direct supervision of the Practice Development Manager, the Patient Coordinator will be responsible for managing the clinical support coordination of the patient's care. Primary responsibility for scheduling follow up appointments, surgery coordination and scheduling, communication across care teams and other Summit departments. Provides other non-clinical activities, in support of patient care, as directed by the physician and/or other provider/clinician.

This is a full-time float position. Monday - Friday schedule of 7:30 AM to 4:00 PM (subject to change based on clinic and/or team needs). Primary locations are TBD at this time. This position may also involve coverage at Summit outreach sites, as applicable.

Responsibilities
  • Answer and respond to patient phone calls, manage team voicemail and email, and triage patient phone calls on a daily basis as appropriate.
  • Manage one or more physicians’ daily schedules, clinic and surgery schedules. Responsible for scheduling and logistics for urgent needs patients.
  • Support coordination of patient care between team members, other service line teams and/or other Summit departments.
  • Communicate information from physicians and other care providers to patients and other staff, in support of patient care.
  • Maintain and complete worklist, in coordination with other identified care team members. Accurately enter physicians’ orders into electronic medical record.
  • At direction of physician or other care providers, may complete standard (non-complex) patient forms such as FMLA, Short Term Disability, Workers Compensation, and other designated requests. Provide patients with information packet and/or documents in accordance with physician protocols.
  • Act as immediate back-up to other teams, including own service line or other service line teams, as assigned. Assist with voicemail, email and task lists as needed.
  • May be responsible for rooming patients and initiating visit for physician and other providers/clinicians, as directed.
  • Perform other duties as assigned.
Benefits

Summit's hiring range for this position is $21.62 to $27.03 per hour. The hired candidate may be eligible to receive additional compensation in the form of bonuses, differentials and/or deferred compensation. Qualifications

Education: High School (required). Associates of Medical Assistant (preferred).

Experience: 2 years: Associate degree or completion of a community or technical college program such as medical administrative, health care specialty, or another related program. In lieu of education, an equivalent combination of coursework and previous related experience may be considered. (required)
