Job summary

Giving Home Health Care, LLC is seeking a full-time Patient Coordinator in Las Vegas to join our Case Management team. The role focuses on ensuring effective communication between our healthcare team and patients, with travel to patient homes and medical offices as needed.

The ideal candidate will have 1–3 years of experience in a care team or patient navigation role, strong communication skills, and a commitment to patient-centered care. Local travel and proficiency with MS Office are required.

Qualifications

  • Strong work ethic and relationship-building skills.
  • Self-motivated with the ability to manage time appropriately.
  • Strong communication skills inside and outside the office.
  • Experience with MS Office products (Outlook, Word, Excel).
  • High school diploma or equivalent is required.
  • 1–3 years of experience in a Care Team or Patient Navigation role.
  • Dependable transportation to travel locally between locations including patient homes, physician offices and Giving offices.
  • Healthcare experience preferred.

Responsibilities

  • Enhance the overall patient experience and ensure patients receive necessary support and resources.
  • Accompany patients to benefit renewal appointments to advocate for the patient and the program.
  • Visit patient homes and update patient care.
  • Communicate information to patients, caregivers, and physicians.
  • Understand HIPAA and maintain confidentiality.

We have an immediate need for a full-time Patient Coordinator to join our Case Management team.

The Patient Coordinator plays a vital role in ensuring effective communication and support between our healthcare team and our patients.

Responsibilities
  • The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources.

$25 - $25 an hour
