Job summary

Altais Medical Group in San Francisco, CA seeks a Board-certified PCP to deliver direct patient care within a team-based, physician-led model. You will coordinate care, promote preventive services, and participate in Chronic Care Management using Epic EHR.

Qualifications

• Active California medical license and board certification.
• Residency in family medicine or internal medicine.
• Current NPI and DEA numbers in good standing.
• Experience with Epic or similar EHR.
• Ability to provide call coverage 24/7 as needed.

Responsibilities

• Provide direct and indirect patient care within a primary care model.
• Promote health maintenance, disease prevention, and health education.
• Coordinate care with multidisciplinary teams and community resources.
• Lead annual health assessments and chronic care management.
• Advise and advocate for cost-effective patient care.
