Qualifications

Responsibilities

• Assess patient’s physiologic health status and review charts.
• Coordinate plan of care with physicians and health team.
• Implement care plans and ensure safety and quality of care.
• Educate patients and families about procedures and discharge plans.
• Collaborate with care team and participate in discharge planning.
