Job summary

LiveWell Homecare Agency, LLC is seeking a Physical Therapist to provide patient-centered, skilled therapy in a supportive rehab environment. You will evaluate patients, develop treatment plans, and deliver therapy to improve functional status and discharge readiness.

Collaborate with patients, families, and healthcare teams, delegate tasks to assistants, and document care activities, ensuring safety and quality of care.

Qualifications

• Bachelor's Degree in Physical Therapy required; Master's Degree preferred.
• Licensure as a Physical Therapist.
• Experience is not mandatory but preferred.
• Strong communication and collaborative skills.

Responsibilities

• Establish and implement treatment plans considering discharge needs and equipment.
• Educate and coordinate with patients, families, and healthcare providers, making referrals when needed.
• Delegate interventions appropriately, ensuring competency and safety.
• Complete accurate documentation in line with departmental policies.
• Conduct assessments, reassessments, and provide consultations as required.
