Job summary

Providence at Home with Compassus seeks a Patient Transition Coordinator to coordinate referrals to hospice, home health, home infusion, and palliative services, ensuring timely access and alignment with Compassus values. You will organize medical records, confirm SOC, and collaborate with Care Transition Coordinators to support families and referral sources with clear communication.

Qualifications

Bachelor’s degree in a health-related field preferred; equivalent combination of education and experience will be considered.
Minimum of one year of experience in a related health care setting preferred.
One year experience in home health, hospice or palliative preferred.
Prior experience with electronic medical records preferred.
Knowledge of home health, hospice, and palliative eligibility requirements preferred.

Responsibilities

Organize and track medical records for patient transitions to home health providers and confirm SOC.
Identify post-acute care needs and collaborate with the Care Transition Coordinator.
Coordinate with Compassus/Ascension at Home intake teams.
Complete the transition of care, including referral submission and outside placement if needed.
Handle large volumes of information with accuracy.
Provide exceptional service to patients, families, and referral sources.
Manage confidential information and produce general correspondences.
Utilize MS Word, Excel, Access, and PowerPoint effectively.
Perform other duties as assigned.
