PURPOSE AND SCOPE:
Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse, in accordance with organization policies, procedures, training, and regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assists in maintaining a safe and clean working environment. Supports the organization's commitment to the Quality Enhancement Program (QEP) and CQI activities, including those related to patient satisfaction, and actively participates in process improvement activities that help patients achieve the organization's Quality Enhancement Goals (QEP).
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Related
- Assist other healthcare team members in providing patient-specific education regarding adequacy measures such as Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and disease process/access.
- Welcome assigned patients, inquire about their wellbeing since their last treatment, and evaluate vascular access for patency.
- Perform vascular access cannulation and administer heparin as delegated or permitted by law.
- Obtain pre- and post-treatment vital signs and weight, and evaluate vascular access before treatment.
- Initiate dialysis treatment following prescribed orders, monitor response, and evaluate for intradialytic problems, providing interventions as prescribed.
- Record vital signs, treatment parameters, and other relevant data accurately and timely.
- Discontinue dialysis according to procedures, evaluate patient before venous access removal, and ensure proper hemostasis and dressing application.
- Perform and document pre- and post-dialysis assessments, including weight and vital signs, with initial patient identification.
- Ensure safety alarms are enabled, including Narrow Venous Limits (NVL) 160.
- Report significant changes in patient condition to the registered nurse or supervisor.
- Monitor vascular access pressures and intervene as needed, documenting findings and actions.
- Prepare laboratory specimens, ensure blood spill disinfection, and manage supplies efficiently.
- Supervise patients performing self-care under RN supervision.
Staff Related
- Assist in training direct care staff on relevant programs under RN supervision.
Education/Communication
- Follow all organizational policies, procedures, and ethical practices.
- Maintain a clean and safe environment, disinfect equipment between treatments, and prepare dialysis systems.
Record Keeping
- Participate in CQI activities and document treatment data accurately in the clinical system.
- Review treatment sheets for completeness, ensure signatures are obtained, and correct omissions.
- Prepare patient charts, lab requisitions, and handle lab specimens properly.
Infection Control
- Support infection control audits and practices, participate in education, and adhere to policies.
Transitional Care Unit
- Follow new patients through their first 4 weeks of dialysis, providing education and coordinating care.
- Assist in patient understanding of home dialysis options and facilitate modality transition.
- Manage supplies and equipment in the TCU.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
- Perform tasks involving heavy lifting, prolonged standing, bending, and moving patients, with assistance as needed.
- Work environment is characteristic of a healthcare setting with exposure to infectious materials.
- Travel between facilities and community locations may be required.
EDUCATION AND LICENSES:
- High school diploma or G.E.D. required.
- Must meet CMS-approved certification requirements within the specified timeline.
- Maintain appropriate state licensure, education, and training as required.
EXPERIENCE AND REQUIRED SKILLS:
- Previous patient care experience preferred.
- Successful completion of dialysis training and CPR certification required.
