Job summary

Avera Creighton Hospital in Creighton, NE is seeking a Licensed Practical Nurse (LPN) to provide patient-centered care under the supervision of RN/APRN/physician. The LPN collects data, performs focused nursing assessments, and collaborates with the care team to develop and modify patient-focused care plans.

Rotating shifts 7pm-7am and 7am-7pm, 72hrs/2wks, every 3rd weekend/holiday. LPN pay scale up to $33.75/hr; sign-on bonus up to $15,000 for eligible candidates.

Qualifications

• Active LPN license in the state of practice.
• Visual acuity and effective communication to perform duties.

Responsibilities

• Collect data and conduct focused nursing assessments.
• Develop and modify patient-centered care plans.
• Implement nursing interventions within the care plan.
• Evaluate responses to interventions.
• Maintain safe and effective nursing care.
• Advocate for patient interests.
• Communicate and collaborate with patients and health care team.
• Assist with health counseling and teaching.

Position Highlights

PCT Pay scale: $18.00 - $20.75 - $23.50

LPN Pay scale: $24.50 - 29.13 - 33.75

A Brief Overview

The LPN practices under the supervision of a registered nurse, advanced practice registered nurse, licensed physician, or other health care provider. The LPN organizes and directs those aspects of care for which he/she is accountable; communicating and coordinating care with well-defined nursing diagnoses, including all health team members.

What You Will Do

• Collects data and conducts a focused nursing assessment of the health status of a patient.
• Participates with other health care providers in the development and modification of the patient-centered health care plan.
• Implements nursing interventions within a patient-centered health care plan.
• Assists in the evaluation of responses to interventions.
• Provides for the maintenance of safe and effective nursing care rendered directly or indirectly.
• Advocates for the best interest of the patient.
• Communicates and collaborates with patients and members of the health care team.
• Assists with health counseling and teaching.
• Delegates and assigns nursing interventions to implement the plan of care.
