Job summary

Avera is seeking a Licensed Practical Nurse (LPN) to float across Behavioral Health Inpatient units, The Link, BHS Urgent Care, and Adolescent Addiction Unit. You will provide patient-focused care within a dynamic team, applying autonomous nursing practice and clinical judgment in a fast-paced setting.

The role requires an active LPN license in the practicing state, strong communication, and the ability to perform essential duties with visual acuity.

Qualifications

Responsibilities

• Collect data and conduct focused nursing assessments
• Develop and modify patient-centered care plans with the care team
• Implement nursing interventions within the care plan

• Maintain safe and effective nursing care and advocate for patients
• Communicate and collaborate with patients and health-care team
• Provide health counseling and teaching to patients and families

The pay range for this position is listed below. Actual pay rate dependent upon experience.

• $24.50 - $33.75

A Brief Overview

The LPN practices under the supervision of a registered nurse, advanced practice registered nurse, licensed physician, or other health care provider. The LPN organizes and directs those aspects of care for which he/she is accountable; communicating and coordinating care with well-defined nursing diagnoses, including all health team members.

What You Will Do

• Collects data and conducts a focused nursing assessment of the health status of a patient.
• Participates with other health care providers in the development and modification of the patient-centered health care plan.
• Implements nursing interventions within a patient-centered health care plan.
• Assists in the evaluation of responses to interventions.
• Provides for the maintenance of safe and effective nursing care rendered directly or indirectly.
• Advocates for the best interest of the patient.
• Communicates and collaborates with patients and members of the health care team.
• Assists with health counseling and teaching.
• Delegates and assigns nursing interventions to implement the plan of care.
