Qualifications

Responsibilities

• Provide patient care based on physical, psychosocial, education, safety and related criteria.
• Communicate with patients, nursing staff and health care team regarding patient status.
• Participate in the care plan from admission to discharge for safe transitions and continuity of care.
• Engage in interdisciplinary collaboration and unit/department PI activities.
