Job summary

Community Bridges in Phoenix seeks a Patient Care Technician (PCT) to join their multi-disciplinary treatment team providing direct care to behavioral health patients. You will monitor safety, implement care plans, and assist with clinical documentation.

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Job Summary

The Patient Care Technician (PCT) is a member of the multi-disciplinary treatment team that provides direct care to behavioral health patients working under the medical direction of a Registered Nurse (RN) in person or telephonic. The primary focus is direct patient care as directed and delegated by the RN, Lead PCT, Operation Supervisor, Operations Administrator, or Program Manager. The PCT is also responsible for the safety of the unit they are assigned to, including identifying environmental and patient-specific safety concerns, and elevating those concerns to the RN, RN Manager, Operations Supervisor, or designee as needed.

Responsibilities
  • Provide direct patient care under the supervision of a Registered Nurse and other clinical leaders as assigned.
  • Monitor patient safety and environmental conditions; identify safety concerns and report them to the RN or designee.
  • Support the treatment team in implementing care plans and crisis interventions as appropriate.
  • Assist with routine clinical tasks and documentation to ensure quality care.
Qualifications
  • High school diploma or GED required; Associate degree or higher preferred.
  • Minimum of 6 months of health care-related experience with exposure to behavioral and medical crisis situations.
  • Minimum of six months of recovery from substance use and/or mental health disorders preferred.
