Job summary

Central City Concern is seeking a Licensed Medical Provider (NP or PA) to join our primary care team at Blackburn Center in Portland, OR. The role involves treating a homeless and underserved population with primary care, mental health, and substance use disorder services, including low barrier prescribing and harm-reduction approaches.

The successful candidate will hold Oregon licensure, have BLS, and meet CCC privileging requirements.

Qualifications

• Graduate of an accredited Nurse Practitioner Program with licensure in Oregon or eligibility.
• Graduate of an accredited Physician Assistant Program with NCCPA certification or eligibility.
• Must have current BLS certification and privileging requirements as required by FTCA.

Responsibilities

• Deliver comprehensive, patient-centered primary care with attention to mental health and substance use disorders.
• Take health histories, perform exams, review labs, prescribe medications, and manage treatments.
• Triage and manage acute illnesses and coordinate care with the CCC team.
