Job summary

Neighborhood Health Center in Portland, OR seeks a Family Nurse Practitioner to deliver primary care on-site within a collaborative clinical team. The role involves assessing, diagnosing, treating, and educating patients with support from physicians and other clinicians.

Requirements include an MS in Nursing, national certification, and an active Oregon prescriptive authority license, plus BLS/ACLS. The position offers full-time hours and a comprehensive benefits package.

Responsibilities

  • Conducts physicals examinations and mental health assessments for preventive, acute and chronic care.
  • Performs, orders, and interprets diagnostic procedures and tests.
  • Develops and implements treatment plans with prescribed medications and injections.
  • Documents findings and updates medical records and care plans.
  • Collaborates with multidisciplinary teams to provide evidence-based care.
  • Provides evening and weekend on-call coverage when scheduled.
  • Participates in team huddles and quality improvement workflows.
  • Attends meetings and participates in committees as requested.
