Job summary

Jobtailor seeks a bilingual care navigator in Monterey County to support CALAIM Enhanced Care Management and Community Support program clients. You will work with care teams and partners to connect individuals with essential resources and ensure ongoing engagement in medical care.

Qualifications

• Fluent in English and Spanish, spoken and written.
• Resident of Monterey County, CA.
• Experience serving homelessness or substance abuse communities.
• Capable of maintaining professional boundaries while providing supportive services.
• Ability to assess and refer mental health or psychosocial needs.
• Maintain clear client case records in a database.
• Associate’s or bachelor’s degree in social or human services preferred.
• Valid California driver’s license and auto insurability.
• Must pass a DOJ criminal background check.
• Community outreach experience: 1 year minimum.
• Willingness to travel ~25%.

• Serve as a contact point for patients and partners; connect to resources.
• Provide health education to promote self-management.
• Schedule and attend primary care visits to review care plans with the Care Team.
• Maintain client case records in the database.

Job description

• Provide telephonic and field-based care navigation services to clients enrolled in the CALAIM Enhanced Care Management and Community Support Program
• Collaborate with Care Coordinators, Community Health Workers, Program Directors, healthcare providers, Care Team members, partners, and vendors
• Build strong relationships with clients to keep them engaged in medical care
• Remove barriers to healthcare resources and services
• Conduct community outreach from Monterey to Salinas
• Establish close relationships with partners and serve as a point of contact for patients
• Provide health education to promote patient self-management
• Identify and connect clients to transportation, housing, food, and other social service resources
• Schedule and attend primary care physician appointments to review and update care plans with the Care Team
• Maintain client case records in the database

Requirements

• Fluency in both English and Spanish, spoken and written
• Candidate must reside in Monterey County
• Prior work experience within homelessness, domestic violence, or substance abuse communities as a resource for assistance
• Ability to establish and maintain personal and professional boundaries while providing supportive services
• Ability to assess and make appropriate referrals for mental health or psychosocial problems
• Ability to maintain clear and concise client case records in a database
• Associate’s or bachelor’s degree in social or human services preferred
• Valid California driver’s license
• Must meet the State’s automobile insurability requirement
• Must pass a DOJ criminal background check
• Community outreach experience: 1 year required
• Willingness to travel 25% preferred
• Must be able to perform light to moderate lifting
• Occasional evening and weekend work may be required
