Job summary

Allegiance Healthcare - Northeast Hospice in Ruston, LA is seeking a Registered Nurse with leadership experience to join our clinical team. This on-site role requires Louisiana RN licensure and at least two years of RN experience in hospice or home care, with supervisory background.

You will review patient information, coordinate care, and support clinicians. The position also involves hiring, education programs, community outreach, and maintaining the electronic health record.

Qualifications

  • Registered nurse with current licensure with no restrictions to practice professional nursing in the state.
  • Must have two (2) years of experience as a Registered Nurse.
  • Previous experience in hospice/home care setting and with two years of management or supervisory experience.
  • Computer experience including knowledge of Microsoft Suite and/or Google Suite.

Responsibilities

  • Review patient information related to hospice/home care needs and assign personnel.
  • Review and evaluate cases and guide clinicians to improve care; respond during operating hours as needed.
  • Review patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
  • Assist clinicians in establishing immediate and long-term therapeutic goals and care plans.
  • Attend case conferences to coordinate care and interdisciplinary involvement.
  • Assist in screening and interviewing new employees and their orientation.
  • Assist in planning and evaluating in-service and continuing education programs.
  • Assess community health needs and participate in outreach to promote the organization.
  • Promote customer service orientation to hospice personnel.
  • Participate in on-call rotation as needed.
  • Maintain documentation in the electronic health record and adhere to policies.

Job Responsibilities:

  • Review available patient information related to the case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to the case as needed. Conferences with Medical Director regarding any questions about an individual's eligibility for services.
  • Reviews and evaluates each case through a variety of means such as home visits, conferences, record review, and the services provided by clinicians; discusses and verifies impressions, instructs, and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.
  • Reviews the patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Assists in the screening and interviewing process of new employees and makes recommendations for the employment of individuals. Assists in the orientation of new employees.
  • Assists the Director of Clinical Operations and other supervisory hospice personnel in the planning, implementation, and evaluation of in-service and continuing education programs.
  • Assesses health needs of the community and assists in the development of the plan(s)/ program(s) to meet needs. Participates in public relations and community activities that promote the organization’s role as an effective member of the hospice and health care delivery system.
  • Promotes customer service orientation to all hospice personnel.
  • Participates actively in quality assessment performance improvement program.
  • Participates in the on-call rotation as needed.
  • Performs other duties and activities as delegated by the Director of Patient Care and/or Administrator.
  • Knowledge of and adherence to all policies and procedures.
  • Maintains compliance by documenting accurately and timely within the organization’s electronic health record.

Education and Experience:

Other Physical Requirements:

  • Ability to wear Personal Protective Equipment (PPE) (Medical Mask)
  • Primary office work with fieldwork visiting patients in their homes, physician offices, hospitals, hospices, nursing homes, etc.
