Responsibilities

The Home Health Patient Care Manager Assistant (LPN) supports the Patient Care Manager by coordinating staff to ensure high-quality, efficient care aligned with physician's orders and regulatory guidelines. Responsibilities include:

• Ensuring team commitment to quality care
• Preparing patient lists and documentation for weekly case conferences
• Providing direct patient care within scope of practice and physician orders
• Assisting with clinician documentation reviews and audits

Education and Experience

License Requirements:

• Current CPR certification
• Valid LPN licensure in the state of practice
• Valid driver's license, vehicle insurance, and access to reliable transportation
