Summary

We are hiring for an LPN Assistant Manager.

This position will be mainly office-based, Monday through Friday, with occasional patient care.

Responsibilities
  • Assist with phone calls from field staff, physician offices, referral sources, and patients. Take messages and orders as needed.
  • Monitor the status of incoming faxes and report relevant information to appropriate personnel.
  • Coordinate communication and fax applicable documents to physician offices.
  • Monitor the status of pending labs, facilitate follow-up, and communicate results to physicians and other personnel as appropriate.
  • Monitor the status of patients in the hospital to ensure proper care coordination.
  • Assist the Patient Care Manager with managing software system reports for effective care coordination and utilization management to promote positive patient outcomes (e.g., therapy utilization, missed visits, rescheduled visits).
  • Assist with payer source authorizations, as needed.
  • Follow up on health check calls or tele-monitoring alerts to ensure proper communication within the team.
  • Prepare patient lists and necessary documents for weekly case conferences.
  • Provide direct patient care as necessary, in accordance with scope of practice, care plans, physician orders, and in coordination with the healthcare team. Do not alter the plan of care.
  • Complete required learning courses and additional assignments as requested by the Executive Director, including state-specific training per regulations.
  • Assist in clinician documentation reviews and audits to ensure accuracy and completeness.
  • Participate in the agency's Episode Management process.
  • Perform other duties as assigned.
Education and Experience

License Requirements:

  • Current CPR certification required.
  • Current LPN licensure in the state of practice.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
