Qualifications

Overview

Would you like to take your nursing career to the next level and lead a team that is passionate about end-of-life care? Heartland Hospice is hiring a full-time leader to support and manage our team.

We're looking for a Patient Care Manager to join our team. You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager. Your responsibilities include maintaining a high-performing hospice program by managing, overseeing, and directing patient care activities to ensure the delivery of quality care. This role involves teaching, evaluating, and developing associates, and ensuring adherence to company policies, procedures, and compliance with local, state, and federal regulations.

  1. Oversee and manage visit requests, including rescheduled, declined, missed, and reassigned visits, making staffing and priority decisions based on clinical judgment.
  2. Respond promptly, professionally, and compassionately to phone referrals and inquiries, including triage of medical concerns within scope of practice.
  3. Prepare for, serve on, and facilitate the Interdisciplinary Group (IDG), including presiding over meetings, presenting patient reviews, coordinating minutes, reviewing patient charts for condition changes, and soliciting input from care team members.
About You
  • Minimum of three years’ experience in direct patient care and at least two years’ experience in hospice or home-based care settings.
  • Graduate of an accredited nursing school; bachelor’s degree in nursing preferred.
  • Current Registered Nurse licensure in the state of residence and requested employment.
  • Knowledge of hospice principles, practices, standards, regulations (Medicare, Medicaid, JCAHO, ACHC), company policies, and understanding of terminally ill patients and families.
  • Ability to apply knowledge of hospice patient and family needs from diverse socioeconomic backgrounds to provide appropriate advocacy and oversight.
