Job summary

Avita Health System in Ontario, OH is seeking a Patient Care Coordinator (PCC) to join Nursing Administration. The PCC will assess, plan, implement, and evaluate patient care while ensuring seamless shift-to-shift continuity and supervising nursing staff.

Ideal candidates hold an Ohio RN license with BLS/ACLS/PALS and Neonatal Resuscitation certs, plus 2+ years of nursing experience. BSN preferred; opportunities for growth and comprehensive benefits are offered.

Qualifications

• Current licensure as a Registered Nurse in the State of Ohio.
• Current BLS, ACLS, PALS certifications.
• Neonatal Resuscitation certification.
• Two (2) or more years of previous nursing experience.

Responsibilities

• Assess, plan, implement and evaluate the nursing care of patients within the nursing units.
• Ensure continuity of care between shifts by supervising care given by nursing assistants, LPNs, and supportive staff.
• Participate in patient and family teaching.

We’re currently seeking a dedicated Patient Care Coordinator (PCC) to join our Nursing AdministrationDepartment at our Ontariolocation.

Position Overview

Assesses, plans, implements, and evaluates the nursing care of patients within the nursing units. Responsible for ensuring continuity of care of the patients between shifts by providing direct care as well as supervising the care given by nursing assistants, LPNs, and supportive staff members. Participates in patient and family teaching. Maintains role as patient advocate with a focus on the organization’s mission.

Qualifications

Required

• Current licensure as a Registered Nurse in the State of Ohio.
• Current BLS, ACLS, PALS certifications.
• Successful completion of Neonatal Resuscitation certification.
• Two (2) or more years of previous nursing experience.

Preferred

• Previous management/supervisory experience.
• Bachelor’s degree in nursing (BSN).
