Job summary

Bridge Home Health & Hospice is seeking a Patient Care Coordinator to support the Director of Patient Care Services in day-to-day clinical operations for the branch. This role trains new clinical staff, supports quality assurance, and acts as a resource for field staff, ensuring a seamless hospice care process.

The position requires coordinating training, auditing patient records, and ensuring adherence to physician plans of care, CMS regulations, and scheduling.

Qualifications

  • Basic understanding of human resource management principles and practices.
  • Computer Skills including knowledge of relevant software.
  • Excel knowledge is strongly desired.
  • Basic knowledge of office machines.
  • Ability to delegate tasks and manage subordinates.
  • Ability to establish and maintain effective working relationships.
  • Ability to meet the public and staff as a positive, friendly, and professional representative of the agency.

Responsibilities

  • Coordinate training for new hires including EMR training, orientation books, and clinical supplies.
  • Assist Quality Assurance with chart audits and auditing compliance for patients.
  • Report patient care summaries to Case Managers with the Director of Patient Care Services.
  • Attend interdisciplinary group meetings and document reviews per CMS regulations.
  • Maintain comprehensive patient documentation in the EMR system.
  • Ensure staff follow physician plans of care orders.
  • Coordinate with Scheduling for staff utilization and start-of-care tasks.
  • Assist with QA of referral documentation.
  • Approve and track branch supply orders for clinician use.
  • Communicate schedules with triage and on-call staff.
  • Obtain orders for intake and perform random QA of referrals.
  • Link patients to community resources as needed.
  • Maintain documentation per policies and laws.
  • Participate in QA/Performance Improvement programs and training.
  • Lift and move supplies as needed.
  • Perform other duties as assigned.

Job description

The Patient Care Coordinator is responsible for assisting the Director of Patient Care Services/ in day-to-day clinical operations for the branch to ensure a seamless and organized hospice care system. The Patient Care Coordinator maintains a comprehensive working knowledge to assist in training new clinical staff, and to serve as a resource for field staff.

Essential Job Functions/Responsibilities:
  • Coordinates training for new hires; to include EMR training, orientation books, and clinical supplies.
  • Assists Quality assurance with chart audits, clinical authorizations review and auditing compliancy for patients.
  • Works with the Director of Patient Care Services and staff clinicians to gather, collate, and report patient care summary information to Case Managers.
  • Regularly participate in interdisciplinary group meetings and ensure through attendance and record reviews that meetings are conducted in accordance with Agency and CMS regulations.
  • Maintains comprehensive working knowledge of patients and ensures the needed documents are received and organized in the system.
  • Works with all members of the clinical team to ensure that they are following the physician plan of care orders.
  • Works with the Director of Patient Care Services and Scheduling teams to ensure proper staff utilization and confirms that consents and NMNOC’s and all other assigned tasks are plotted correctly on start of care.
  • Assists with Quality Assurance of referral documentation.
  • Approve and track supply orders for the branch to ensure availability of sufficient inventory for clinicians.
  • Communicates schedule with triage and on-call staff.
  • Obtains orders for intake and completes a random examination for Quality Assurance of patient referrals.
  • Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by the company.
  • Maintain current documentation on EMR system according to organization policies/procedures and applicable laws/regulations.
  • Participation in Quality Assurance/Performance Improvement Program and Bridge Hospice sponsored training.
  • Able to lift supplies and equipment. Considerable reaching, standing, stooping, bending, and/or crouching. Visual acuity and hearing to perform required PCC skills.
  • Other duties as assigned.
