Job summary

Responsibilities

• Assesses eligibility, benefits, and education for admissions.
• Reviews and enters authorization data and documents case in medical files.
• Coordinates resources to assure continuity and quality of care.
• Attends case management rounds and reviews authorizations and diagnoses.
• Develops disposition plans in collaboration with patient and care team.
• Identifies barriers to goals and ensures discussion with care team.
