Job summary

Responsibilities

• Implements Continuing Care programs and integrates into daily practice.
• Provides case management to targeted patients/families; develops work plans and coordinates services.
• Monitors high-risk members and assists access to care; evaluates outcomes and recommends changes.
• Assists patients/families with identified problems/concerns.
• Meets with Continuing Care providers and PCP Team to monitor patients and coordinate community care.
• Makes hospital rounds for UM and discharge planning; assists with transfers home or to alternative facility.
• Reports issues to management and ensures regulatory requirements are met.
• Acts as contact for MOA/Oahu discharges; coordinates smooth transfers.
• Participates in community hospital interdisciplinary team meetings.
