Qualifications

• Provides care appropriate to any age-related needs of the patients. Assess and interpret data about the patient's status in order to identify and provide the appropriate care.
• Plan of care developed and customized based on nursing assessment to include medical and nursing diagnosis, patient specific problems and needs under the supervision of the RN.
• Facilitates patient and family involvement, discharge planning, teaching needs, measurable outcomes are included in plan of care.
• Maintains knowledge of home health and legislative issues impacting the future of home care.
• Completes all required documentation according to Agency policies; collaborates with all personnel through patient Team Conference.

Responsibilities

• Assesses patient status and identifies appropriate care.
• Coordinates admissions and nursing assessments and supervision activities.
• Participates in on-call process and ensures care quality.
• Reviews patient charts for compliance with home health requirements and policies.
• Ensures completion of in-home competency checks of field staff and acts as Administrator when needed.

Education

Associates in Nursing - Required

Professional License with State of Practice - Required

Basic Life Support (BLS) - Required

3-5 Years Acute Care or Home Health setting - Preferred
