Responsabilités

Assess and document physical, psycho-social, and functional status; develop and implement care plans.
Coordinate with the health care team to ensure timely, patient-centered care.
Administer medications with adherence to the 5 rights and safety protocols.
Lead and mentor staff; communicate effectively with physicians using SBM (Situation, Background, Assessment, and Recommendation).
Educate patients and families to promote understanding and healthier outcomes.
