Job summary

Ennoble Care in Voorhees, NJ, seeks a full-time Patient Care Coordinator to join our office-based clinical care team. You will support care coordination, documentation, and communication with providers and care teams.

The role emphasizes proactive collaboration, accurate record-keeping, and timely follow-up to ensure continuity of care across settings. Monday–Friday, 8:00am–4:30pm EST; benefits available.

Qualifications

  • Must be legally eligible to work in the United States.
  • Active clinical certification or licensure (PCT/CNA/MA/CMA/LPN/RN or comparable).
  • Minimum 1 year of clinical/administrative or care coordination experience in healthcare.
  • Proficient with Microsoft 365 and internal systems.

Responsibilities

  • Coordinate patient-centered care activities using the EMR system.
  • Support providers and clinical teams with documentation and follow-up.
  • Serve as a liaison between providers and care teams on updates and priorities.
  • Communicate patient needs and urgent concerns to the clinical team promptly.
  • Assist with care transitions and referrals as needed.
  • Maintain accurate documentation per workflows and escalation protocols.

Ennoble Care is seeking a full-time Patient Care Coordinator to join our team in an office-based setting in Voorhees, New Jersey. This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice-style model of care.

While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.

Key Responsibilities
  • Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system
  • Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination
  • Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities
  • Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner
  • Assist with care transitions, referrals, and coordination with external partners, as needed
  • Collaborate closely with fellow Patient Care Coordinators and internal care team members within an office-based environment
  • Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision-making and efficient clinical operations
  • Contribute positively to team collaboration, workflow efficiency, and day-to-day office operations
Requirements
  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure, such as:
    • Patient Care Technician (PCT)
    • Certified Nursing Assistant (CNA)
    • Medical Assistant (MA) / Certified Medical Assistant (CMA)
    • Licensed Practical Nurse (LPN)
    • Registered Nurse (RN)
    • Or comparable credential
  • Minimum 1 year of experience in a clinical and administrative or care coordination role within a healthcare setting
  • Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and internal systems
  • Cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence
  • Well-developed organizational, time-management, and attention-to-detail skills, paired with an optimistic approach and initiative-driven mindset
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 8:00am - 4:30pm EST, in an office-based setting in the Voorhees, New Jersey area
    • No weekends
    • No on-call
    • Daytime hours
    • Seven (7) company-paid holidays annually
  • Ability to pass a background check
