We are seeking a compassionate, organized, and relationship‑driven Patient Care Coordinator to support perinatal and postpartum patients within an OB/GYN practice model. This hybrid role works both virtually and in‑person within a collaborating OB/GYN office, partnering closely with providers, clinical staff, and patients to deliver coordinated, patient‑centered care across pregnancy and the postpartum period.
