The Patient Care Coordinator interviews patients and their families coordinating plans, programs, and activities that meet their emotional and social needs. Provides crisis intervention, assisting patients in understanding the implications and complexities of life situations, relationships, and its impacts on lifestyle. Serves as a liaison between the hospital, governmental, and external supports.
