Job summary

Physiofit, a brand partner of Upstream Rehabilitation, seeks a Patient Care Coordinator in Baton Rouge, LA. This role supports clinic growth and efficient patient intake, working with the Clinic Director to execute practice management tasks and maintain a positive patient experience.

The PCC-I will handle scheduling, referrals, deposits, and non-clinical documentation while ensuring compliance training and smooth daily operations in a busy clinic.

• High School Diploma or equivalent.
• Strong communication with Business Office and Field leadership.
• Ability to multitask, manage deadlines, and work with little supervision.
• Team player with efficient time management and presentation skills.

Responsibilities

• Collect all money due at the time of service.
• Convert referrals into evaluations.

Position Summary

The Patient Care Coordinator supports clinic growth through excellence in execution of the practice management role and patient intake processes. This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures. The PCC-I position is responsible for supporting the mission, vision, and values of Upstream Rehabilitation.

A day in the life of an Patient Care Coordinator

Core responsibilities

• Collect all money due at the time of service.
• Convert referrals into evaluations.
• Schedule patient visits.
• Customer Service o Create an inviting clinic atmosphere.
• Make all welcome calls.
• Monitor and influence arrival rate through creation of a great customer experience.
• Practice Management o Manage schedule efficiently.
• Manage document routing o Manage personal overtime.
• Manage non-clinical documentation.
• Manage deposits.
• Manage caseload, D/C candidate, progress note, and insurance reporting.
• Monitor clinic inventory.
• Attend any required training with the Territory Field Trainers (TFT) for Raintree and other business process updates.
• Complete quarterly compliance training.

What experience do you need?

• High School Diploma or equivalent.
• Communication skills - must be able to relate well to Business Office and Field leadership.
• Ability to multitask, organizational detail, ability to meet deadlines, work with little to no supervision.
• As a member of a team, must possess efficient time management and presentation skills.
