Physiofit, part of Upstream Rehabilitation, seeks a Patient Care Coordinator in Lafayette, LA to support clinic growth and patient intake efficiency. You will collaborate with the Clinic Director to execute practice management tasks and foster a welcoming, compliant clinic environment.

Responsibilities include patient scheduling, referrals, deposits, and insurance reporting while maintaining a high level of customer service. This is a full-time role with strong growth potential.

Qualifications

• Strong communication skills with business office and field leadership.
• Ability to multitask and manage details with minimal supervision.

Responsibilities

• Collect all money due at the time of service.
• Convert referrals into evaluations.
• Schedule patient visits.
• Create an inviting clinic atmosphere and handle welcome calls.
• Monitor arrival rate through excellent customer experience.
• Manage the schedule and non-clinical documentation.
• Handle deposits and clinic inventory.
• Manage caseload, discharge candidates, progress notes and insurance reporting.
• Attend quarterly compliance training and trainings with TFTs for updates.

Position Summary:

The Patient Care Coordinator supports clinic growth through excellence in execution of the practice management role and patient intake processes. This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures. The PCC-I position is responsible for supporting the mission, vision, and values of Upstream Rehabilitation.

A day in the life of an Patient Care Coordinator:

• Core responsibilities.
• Collect all money due at the time of service.
• Convert referrals into evaluations.
• Schedule patient visits.
• Customer Service o Create an inviting clinic atmosphere.
• Make all welcome calls.
• Monitor and influence arrival rate through creation of a great customer experience.
• Practice Management o Manage schedule efficiently.
• Manage document routing o Manage personal overtime.
• Manage non-clinical documentation.
• Manage deposits.
• Manage caseload, D/C candidate, progress note, and insurance reporting.
• Monitor clinic inventory.
• Attend any required training with the Territory Field Trainers (TFT) for Raintree and other business process updates.
• Complete quarterly compliance training.

What experience do you need?

• High School Diploma or equivalent.
• Communication skills - must be able to relate well to Business Office and Field leadership.
• Ability to multitask, organizational detail, ability to meet deadlines, work with little to no supervision.
• As a member of a team, must possess efficient time management and presentation skills.
