Job summary

Providence Facey Medical Foundation is seeking a Coordinator Clinical Care to support care coordination within the primary care practice. You will monitor coordination activities, assist clinical teams, and help identify high-acuity patients to ensure coordinated care across the patient panel.

You will collaborate with the Practice Manager and lead physician to shape this role and improve patient outcomes, while upholding the foundation's mission of compassionate, patient-centered care.

Responsibilities

• Monitor care coordination processes and support primary clinical teams.
• Identify high acuity patient population and coordinate care for this group.
• Collaborate with Practice Manager and lead physician to develop this role to best serve the patient panel.

Education

Graduate of school for medical assistants

Diploma/certificate from military in Medical Specialist Course
