  • Assist with the development of effective communication mechanisms for the Care Center Team and its leadership.
  • Work with leadership to manage productivity and quality performance.
  • Coordinate and track all activities under the supervision of leadership to meet deadlines.
  • Research, collect, and analyze data, and present findings to support project activities.
  • Provide additional guidance and support to team members including performance reviews, coaching, training and provide developmental opportunities as needed.
  • Support Quality Assurance team by monitoring and maintaining quality standards to ensure effective delivery of compliance and processes.
  • Supply support to leadership with scheduling and routing errors.
  • Maintains an active program of self-study and learning, to enhance professional skills and remain current with technological changes.
  • Participates in special Monogram Health initiatives and other duties as assigned.
  • Provides excellent customer service to all levels of employees including customers, peers, PODs, clinical and leadership.
  • Act as liaison between Care Center and supporting departments to promote collaboration and give feedback.
  • Engages in team meetings, team building activities, and other opportunities to strengthen the department.
  • Communicates information through appropriate methods, reads and interprets the most complex information, collaborates with teams across departments, writes clearly andinformatively.
Position Requirements
  • Must have 2 years of Engagement Specialist 2 experience with Monogram Health for longer than 90 days
  • Minimum of 30 days in Engagement Specialist II role within Monogram Health
  • High School Diploma or GED required.
  • Experience scheduling any type of visit with a high rate of accuracy.
  • Independent direction and judgement, and critical thinkingskills
  • Ability to inspire and hold othersaccountable.
  • Proficient in current computer applications
  • Excellent oral and written communication skills
