Job summary

Prosperity Wellness Center is seeking an experienced care coordinator to bridge patient care between internal teams and external providers in Tacoma, WA. You will assess aftercare needs and secure referrals and appointments to ensure smooth transitions in the recovery process.

The role emphasizes documenting care, coordinating discharge plans, and maintaining strong relationships with clinicians and care teams.

Qualifications

• 1 year experience in a mental health and/or chemical dependency setting preferred.
• Experience working with clients and documentation in a hospital and/or mental health facility required.
• Supervisory responsibilities: Includes overall accountability for assigned work group relative to operational goals, personnel requirements, and budgetary constraints.
• At least 3 years' experience in care coordination or customer service. Experience in clinical aspects of behavioral medicine preferred.

Responsibilities

• Develop a continuing care plan aligned with treatment team recommendations and patient needs.
• Timely communication and documentation in compliance with HIPAA requirements.
• Establish positive working relationships with clinicians, medical providers, EAPs, and VA accounts.
• Identify and overcome barriers to patient care.
• Provide case management for patient success in current program and next steps in recovery.
