Job summary

Persante Health Care is hiring three part-time Patient Care Coordinators for weekend coverage in Mount Laurel, NJ. Schedule: Saturday and Sunday, 16 hours per week. Remote work location available with shifts from 8:00 AM to 4:30 PM or 11:30 AM to 8:00 PM.

The role supports sleep study scheduling, documentation, and communication to ensure regulatory compliance and a positive patient experience. 2–4 years in customer service and healthcare exposure are preferred, with bilingual Spanish a plus.

• Inform, educate, and respond to patient and physician questions regarding the sleep study process.

The Patient Care Coordinator supports high-quality patient care by serving as a liaison between patients, physician practices, hospitals, support services, and payors. This role is responsible for coordinating sleep study scheduling, documentation, and communication to ensure accurate workflows, regulatory compliance, and a positive patient experience.

Schedule: Saturday and Sunday

Hours: 16 hours per week

Available Shifts: 8:00 AM - 4:30 PM

11:30 AM - 8:00 PM

Essential Duties and Responsibilities

Patient Coordination and Scheduling

• Receive and enter sleep study orders into designated systems, ensuring medical necessity aligns with documented symptoms and physician orders
• Schedule patients for sleep studies in a timely manner and in accordance with company policy
• Obtain and verify demographic and medical information during the scheduling process
• Inform, educate, and respond to patient and physician questions regarding the sleep study process

Communication and Documentation

• Communicate information received from patients, physician offices, and hospitals to appropriate internal teams
• Review and verify source materials to ensure accuracy and completeness in accordance with Medicare and payer standards
• Follow up to correct or complete missing or inaccurate information as needed

Compliance and Operational Support

• Perform duties in compliance with HIPAA guidelines and company policies
• Support cross-training initiatives, including coverage of other Patient Care Coordinator functions such as Medical Records, as assigned
• Perform additional operational tasks to support departmental workflows as needed

Required Qualifications

• Ability to work effectively in a fast-paced environment
• Effective verbal and written communication skills

Preferred Qualifications

• Experience supporting sleep medicine or diagnostic services
• Bilingual (Spanish) proficiency

Physical and Work Requirements

Ability to sit for extended periods and perform repetitive computer-based tasks. Ability to communicate effectively by phone and electronic systems. Ability to work standard business hours in an in-person office environment.
