The Patient Assessment Coordinator (PAC) oversees the completion and transmission of the Inpatient Rehab Facility Patient Assessment Instrument (IRF-PAI) within required timeframes, ensuring compliance with CMS and other regulatory standards. Responsibilities include data collection, documentation, staff education, and acting as a primary resource for IRF-PAI scoring issues. The PAC fosters positive interdisciplinary relationships.

Minimum Qualifications:
  • Graduate of an accredited RN, OT, PT, or SLP program with current Texas licensure
  • At least two years of clinical experience in an acute hospital, rehab, or LTACH setting
  • Experience with IRF PAI or MDS coordination (preferred)
  • Leadership, communication, and interpersonal skills
  • Knowledge of Medicare rehab guidelines and medical terminology
  • Proficiency with Outlook, Excel, Word, PowerPoint, and data management software
Essential Duties:
  • Coordinate data provision for Medicare PPS and quality reporting
  • Manage IRF-PAI data collection, documentation, and staff education
  • Collaborate with physicians for documentation accuracy
  • Monitor coding accuracy and compliance
  • Conduct chart audits and quality documentation reviews
  • Educate staff on Section GG and other CMS guidelines
  • Develop policies and solve problems related to IRF payment systems and reporting
  • Maintain regulatory compliance
  • Support team efforts to optimize patient outcomes and reimbursement
  • Perform administrative tasks such as patient rounding
