Foundations for Living is seeking an Assessment Coordinator for Mayhill Hospital in Denton, TX. Mayhill Hospital, located in Denton, TX, just north of Dallas and Fort Worth, is a 59-bed private psychiatric hospital specializing in mental health and chemical dependency care for adults and seniors. We are among the few facilities in the area capable of providing care to individuals with coexisting psychiatric and medical conditions. Our facility is uniquely trained to care for seniors who have exhausted their psychiatric treatment options. Position Summary
The Assessment Coordinator assists in responding to referrals and requests for services and information. They provide technical support to the admission process under the supervision of the Director of Intake and support the case management program by facilitating appropriate dispositions in accordance with hospital policies and procedures.

Qualifications
Education : Graduation from an accredited nursing program.
Experience : Preferably two or more years in intake assessment within an acute psychiatric setting.
Licensure : Valid license to practice in Texas as an RN, LMSW, LCSW, or LPC.
Additional Requirements : Strong knowledge of DSM-V, ICD-10, Mental Status Exam, and preferably, addiction and recovery fields. CPR certification and training in Handle with Care (HWC) Crisis Intervention are required before department orientation and during restraining procedures. Overtime and flexible hours may be required.
