This entry-level position reports to the Social Worker, may require weekends, evenings, and holidays, and includes collaboration with volunteers and other departments to deliver meaningful activities and monitor

Qualifications

• Knowledge of safety protocols and infection control measures.
• Time management for activity setup and admin tasks.
• Diverse backgrounds and culturally appropriate activities.
• Basic therapeutic techniques such as reminiscence or art therapy.
• Computer software related to activity planning and documentation.
• Regulations and guidelines for long-term care facilities and activities programming.

Responsibilities

• Assists in developing and implementing enriching programs and activities for patients.
• Develops and leads individual and group activities for swing program patients.
• Initiates and conducts activity assessments for new admits and seeks review from senior staff.
• Documents activity participation and tracks outcomes.
• Participates in care planning to coordinate activities with the multidisciplinary team.
• Maintains safe activity environments through risk management and cleaning.
• Assists with scheduling follow-up appointments and transportation for activities.
