Job summary

Tidewell Hospice, part of Empath Health, seeks an RN with hospice experience to support patients primarily by phone and in-clinic visits. The role focuses on coordination of care across pathways, with no home visits required.

Applicants should have an Associate’s degree (A.A.) or equivalent and a current Florida RN license. This hybrid position offers standard business hours in St. Petersburg, FL, and opportunities for ongoing education and career growth.

Responsibilities

• Gathers information on patient condition/status and plan of care.
• Documents referrals to the Pathways team in the EMR.
• Provides resources, education and support to clients, families and care partners.
• Oversees transition of care from Pathways to needed services.
• Communicates with Empath Health physicians regarding patient eligibility for hospice.
• Obtains verbal certification of prognosis (eligibility).
• Assesses need for resources outside the Integrated Network of Care.

Job description

Since 1980, Tidewell Hospice has proudly served families across southwest Florida with compassionate, dignified care. As a member of Empath Health, we help patients with advanced illness live meaningfully while supporting their families with warmth, expertise, and respect.
This is a hybrid position. The employee will be required to report to our St. Petersburg, FL office and will work a schedule of Monday through Friday from 8:30 AM to 5:30 PM.
This role consists of 8-hour shifts and supports the Tidewell and Empath Hospice service regions. The position is primarily focused on providing telephonic patient support and visits; there is no travel to patient homes, and patient interactions are conducted by phone rather than through video conferencing.

What You’ll Do

• Gathers information related to patient’s present condition/status, current and future plan of care, and identifies any barriers to optimal care.
• Documents referrals to the Pathways team in the electronic medical record.
• Provides resources, education and support to clients, family and care partners regarding decision making.
• Advocates for and oversees the transition of care from Pathways to any services needed.
• Communicates with internal colleagues including Empath Health physicians regarding client’s eligibility for services including hospice care.
• Obtains and documents verbal certification of patient’s prognosis (eligibility).
• Assesses the need for resources outside the Empath Health Integrated Network of Care.

What You’ll Need

• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
• Minimum two (2) years of hospice experience.
• Licensure: Current valid Florida RN license.
• Continuing education: As required for licensure.
• Knowledge of hospice regulations such as admission/qualification criteria, reimbursement, and levels of care preferred.
• Excellent, positive relationship building, outreach efforts, problem solving, organizational, time management, creativity, interpersonal skills, written and verbal communication a must.
