Job summary

Empath Health's Tidewell Hospice is seeking an RN - Triage/Field to visit patients in their homes, long-term care facilities, hospice houses or hospitals, providing clinical, educational and emotional support after-hours.

Under the direction of a licensed physician, you assess patients, manage discomfort, and relieve symptoms with palliative care nursing. You’ll document services in the EMR and help update the Plan of Care for the Sarasota and Manatee counties.

Qualifications

• Graduate of approved nursing program and current unrestricted Florida RN license.
• Valid and current CPR certification required.
• Minimum one year experience in an acute-care Hospital environment providing direct patient care; previous experience as a Hospice RN preferred.
• CHPN certification is strongly encouraged within three years of employment.
• BSN or MSN preferred.
• An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.

Responsibilities

• Visit patients and provide clinical, physical, educational and emotional support after-hours.
• Under physician direction, perform patient assessment, manage discomfort and provide symptom relief in palliative care.
• Utilize nursing interventions and teaching to maximize patient comfort and quality of life.
• Attend patient deaths and provide bereavement support to the caregiver/family.
• Document patient services accurately in the EMR reflecting the care plan and outcomes.
• Review and update the Plan of Care, physician orders and MAR to meet patient/family needs.

Job description

Schedule

4 nights/week 12a-8a, alternating weekends shifts.

Coverage Area:

Coverage in Sarasota and Manatee counties; Charlotte and DeSoto service counties as needed.

The RN - Triage/Field

• Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the Triage team (in in the patient home, long term care facility, assisted living facility, hospice house or hospital)
• Under the direction of a licensed physician, performs patient assessment, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.
• Attends patient deaths, provides bereavement support to the patient caregiver/ family, as indicated and performs post mortem care.
• Provides accurate and timely documentation of patient services within electronic medical record to reflect the individualized plan of care and outcomes of interventions.
• Reviews and updates the Plan of Care, physician orders, MAR/Pharmacy profile and other records to ensure patient/family needs are met.
