Job summary

Sea Mar Community Health Centers is seeking a Community Health Worker to connect patients to health and social services in home, community, and clinic settings. You will document interventions, coordinate Pathways activities, and advocate for clients while working with partners and Pathways staff.

This position emphasizes community outreach, cultural liaison work, and helping clients overcome barriers to care, including transportation and access to resources.

Qualifications

• High school diploma or GED required.
• Bilingual English/Spanish preferred.
• Must be computer literate and able to use CCS Platform.
• Valid driver license, proof of auto insurance and a vehicle safe for daily use.
• Annual background check and health requirements (TB, flu vaccine).

Responsibilities

• Work primarily out in the community to reach patients.
• Be a patient advocate and link to community resources.
• Document interactions and assessments in Pathways and Sea Mar’s EHR.
• Attend trainings and meetings for Pathways and Sea Mar.
• Collaborate with HUB partners to coordinate care.
• Provide follow-up and referrals to services as needed.
