Job summary

Cornerstones of Care in Kansas City, MO is seeking a Case Management Specialist-Pathways to deliver contracted case management services. You will manage client cases, attend meetings and court hearings, complete notes, and coordinate with providers.

Starting salary depends on education and experience; this is a full-time position at the Gillis Campus. A valid driver’s license and background checks are required, with on-call rotation.

Qualifications

• Bachelor’s degree in human behavioral science; includes 30 semester or 45 quarter hours in human behavior related fields.
• 1–3 years of experience required.
• Must pass background check, physical and drug screening; valid driver’s license and vehicle insurance.

Responsibilities

• Carry out case management services per contract and licensing regulations.
• Attend meetings and/or court hearings; exercise discretionary judgment in service delivery.
• Complete case notes and data entry; prepare state-required documents on time.
• Assist with organizing independent living skills classes and crisis intervention.
• Coordinate services with providers; participate in on-call rotation.
