Job summary

Greater Cincinnati Behavioral Health Services is seeking a PATH Case Worker to support adults experiencing homelessness in Hamilton County. The role involves engaging clients, supporting housing stabilization, and coordinating with PATH staff to ensure connection to mental health and substance use services.

The position requires a high school diploma or GED and may require driving a personal vehicle. The PATH team emphasizes outreach, chart maintenance, and client engagement, with opportunities

Qualifications

• High school diploma or GED required.
• Previous personal experience with homelessness and/or mental illness.
• Position may require frequent driving of the employee’s personal vehicle.
• Valid driver’s license, vehicle, and insurable driving record may be required.

Responsibilities

• Conducts satisfaction surveys including Ohio Outcomes surveys.
• Follows-up with former PATH clients to confirm housing and mental health service maintenance and to identify re-engagement needs.
• Assists with client engagement and outreach, including in-person visits and calls to last known contacts.
• Provides on-site assistance with service delivery and coordination.
• Provides peer support services to PATH clients and connects them to Recovery Center when needed.
• Maintains outreach log.
• Provides other duties as assigned to support PATH teams.
• Attends weekly PATH meetings as available.
• Performs other related duties as assigned.
