Job summary

Community Health Center in Meriden, CT is seeking a PATH Care Coordinator to manage service coordination for individuals enrolled in Tier Two of PATH. You will develop individualized care plans, link clients to medical, housing, and social resources, and monitor recovery progress.

The role emphasizes client intake, coordinating treatment, and maintaining confidential records in line with HIPAA and regulations. Bilingual preferred; 23–27 USD/hr compensation, full-time position.

Qualifications

• Bachelor's degree in a related field.
• Experience in outreach and care coordination.
• Experience in substance use field.
• Ability to communicate effectively with hard-to-reach populations.

Responsibilities

• Develop and implement individualized service plans linking clients to medical, housing, mental health, and legal support systems.
• Conduct initial evaluations and ongoing screenings to determine client needs and treatment readiness.
• Track client progress, adjust care plans as needed, and advocate for clients with healthcare professionals and state agencies.
• Maintain accurate, timely, and confidential patient charts in compliance with HIPAA and facility regulations.
• Conduct internal and external outreach to patients and community partners to raise awareness about PATH SUH services and recruitment.
• Work with PATH Project Director and Evaluator on evaluation goals.
• Participate in CKP, Interdisciplinary and Quality Improvement Meetings.
