Job summary

Qualifications

• Knowledge of interactions with children and families.
• Understanding of wraparound services and case management.
• Able to manage a caseload and document activities.

Responsibilities

• Conducts client intakes for PAT participants.
• Establishes eligibility for PAT Wraparound Services.
• Enters and monitors information in the case management database and Visit Tracker.
• Provides parent support and education on child development.
• Maintains records and prepares monthly reports.
