Job summary

Qualifications

Responsibilities

• Go to nursing facilities and work with PASRR clients to achieve goals.
• Provide personal assistance to doctor visits, community events, etc. as needed.
• Input data into system; manage time and attend plan of care meetings and reviews.
• Travel to counties to provide service; transportation provided by the agency.
• Provide nurturing attention to individuals with disabilities in nursing facilities.
• Complete goals on Plan of Care and communicate effectively with staff, clients, and supervisors.
