Job summary

Bluebonnet Trails Community Services seeks a Habilitation Coordinator to advance PASRR services for adults with IDD in nursing facilities or community settings. You will assess needs, coordinate services, advocate for independence, and guide transition planning with families and providers.

Location includes Guadalupe, Gonzales, and Caldwell Counties; strong collaboration and independent work are essential for success in this role.

Qualifications

• Bachelor's degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, or early childhood education/intervention.
• At least one year experience with individuals with IDD is required; degree field not in human services requires 24 college credits in Human Services.
• Meet driving requirements and maintain approved driving status throughout employment.

• Complete intake and assess needs.
• Coordinate specialized services and advocacy for community living.
• Support transition planning whether in nursing facilities or community.
• Facilitate collaboration with families, providers and community partners.
