Job summary

Qualifications

• Bachelor’s degree in social work, psychology, nursing, or related field.
• Must meet Qualified Mental Health Professional (QMHP) requirement.
• Three years of full-time work experience in human services case management.
• Valid Texas driver's license with a clean driving record and proof of insurance.
• Strong organizational and communication skills.

Responsibilities

• Conduct PASRR Level I and Level II screenings for nursing facility admissions.
• Collaborate with healthcare providers, social workers and families to plan services.
• Monitor care plans to ensure PASRR compliance and update as needed.
• Maintain accurate records of assessments and communications per regulations.
• Educate facility staff and families on PASRR processes and resources.
• Coordinate with state agencies to facilitate timely service delivery.
• Advocate for residents' rights throughout the care continuum.
