Job summary

Camino Real Community Services in Texas is seeking a PASRR Case Manager to conduct PASRR Level I and II screenings for seniors and adults in nursing facilities, coordinating care plans and ensuring regulatory compliance. About The Role

The PASRR Case Manager plays a critical role in ensuring that individuals with serious mental illness or intellectual disabilities receive appropriate assessments and services in compliance with the Preadmission Screening and Resident Review (PASRR) federal mandate. This position involves conducting thorough evaluations to determine the level of care needed and coordinating with healthcare providers, facilities, and families to develop and implement individualized care plans. The Case Manager acts as an advocate for the rights and needs of residents, ensuring they are placed in the most suitable care settings and receive necessary supports. This role requires meticulous documentation and reporting to maintain compliance with state and federal regulations. Ultimately, the PASRR Case Manager contributes to improving the quality of life for vulnerable populations by facilitating access to appropriate long-term care services and supports.

Minimum Qualifications
  • Bachelor’s degree in social work, psychology, nursing, or a related human services field.
  • Must meet Qualified Mental Health Professional (QMHP) requirement.
  • Three years of full-time work experience in human services case management.
  • Must have a valid Texas driver's license, a good driving record in the past 3 years, and valid auto insurance coverage.
  • Strong organizational skills with the ability to manage multiple cases simultaneously.
  • Excellent communication skills, both written and verbal.
Preferred Qualifications
  • Master’s degree in social work, counseling, or a related field.
  • Knowledge of PASRR regulations and federal/state long-term care policies.
  • Experience working specifically with individuals with intellectual disabilities or serious mental illness.
  • Familiarity with electronic health records (EHR) and case management software.
  • Training in trauma-informed care or person-centered planning.
